La Madera Fire & Rescue uses pre-incident planning to learn about properties in our response area. Relevant information about buildings, access, utilities, and other site features can help the department prepare for a response.
Use the questionnaire below to share property information. A site plan can be useful if you already have one. If you have questions about what to include, email contact@lamaderafire.org before submitting.
Before You Submit
- This form is for non-emergency department planning. Call 911 for an emergency.
- Include information relevant to the property and emergency access. Do not include passwords, financial information, or unrelated personal documents.
- Submitted details are intended for department planning review. Submission does not arrange a home visit, confirm an inspection, or replace information you should give a 911 dispatcher during an emergency.
- If property details change, contact the department about updating the information.
